SUPPORTED LIVING

A Home of Your Own. Support Built Around the Life You Want.

Supported living helps adults enjoy greater independence while receiving personalised care and practical support with the areas of everyday life where assistance is needed.

At Breathe Care Services, we support individuals to make choices, develop valuable life skills, manage daily responsibilities and participate more fully in their communities.

Whether you are preparing to move away from the family home, transitioning from another care setting or reviewing an existing support arrangement, we begin by understanding the life you want to build—not simply the care tasks you need completed.

UNDERSTANDING SUPPORTED LIVING

Personalised Support Within Your Own Living Arrangement

Supported living is designed for adults who want to live more independently but may still need care, guidance or practical assistance with aspects of everyday life.

Unlike a traditional residential-care arrangement, accommodation and care are generally treated as separate elements. This can give the individual greater choice and control over their home, routines, relationships and support arrangements.

The amount of assistance required will vary from person to person. Some individuals may need a few scheduled hours of support each week, while others may require daily assistance, extended support or overnight care.

Depending on the assessment and agreed support plan, assistance may include personal care, medication support, cooking, shopping, household tasks, budgeting, appointments, community access and developing independent-living skills.

𝐈𝐦𝐩𝐨𝐫𝐭𝐚𝐧𝐭 𝐜𝐥𝐚𝐫𝐢𝐟𝐢𝐜𝐚𝐭𝐢𝐨𝐧 Breathe Care Services provides the care and support element of supported living. Accommodation may be arranged separately through a landlord, housing association, family arrangement or another appropriate provider. The responsibilities of each organisation will be clearly explained before support begins.
